Neighborhood Health Providers
(212) 808-4775
Neighborhood Health Providers (NHP) is a Medicaid-managed care plan that Brookdale, along with Jamaica Hospital Medical Center, owns. NHP is licensed by the New York State Department of Health and boasts affiliations with over eight major hospital/healthcare systems such as Brookdale's.
The NHP network has grown tremendously since its beginnings in 1995. For instance, all six of Brookdale's Family Care Centers (BFCCs) and the Brookdale Pediatric Resource Center are in the NHP system, along with many other primary care givers throughout Brooklyn and Queens.
If you, as an enrolled member, choose a BFCC for your primary care, you'll enjoy the benefits of NHP coverage, along with the beauty of brand-new BFCC facilities and eager, customer-focused staff. Every BFCC is backed by the expertise of the largest nonprofit teaching hospital in Brooklyn--The Brookdale University Hospital and Medical Center.
NHP allows Brookdale to further provide an integrated continuum of care. Today's Brookdale provides care that considers the whole patient and the entire family from birth to old age.
