A Home for Seniors

With its emphasis on long-term geriatric care, the Schulman and Schachne Institute's services include 172 long term beds.  Here, traditional geriatric patients and those with Alzheimer's Disease and related dementias can find a home.  Our staff is dedicated to providing excellent care while respecting our residents' dignity and independence.  The physical and mental well-being of each resident is maximized and supported through the coordinated efforts of interdisciplinary care teams.  Residents and their families are important members of the team.

Services

A comprehensive and creative approach to individual care is initiated by the staff in collaboration with residents and their families.  The interdisciplinary team members that work with residents and their families include a dedicated physician specializing in geriatric care, nurses, a social worker, a recreation therapist, a dietitian, a physical therapist and an occupational therapist. The complete resources of the Schulman and Schachne Institute and The Brookdale University Hospital and Medical Center are available to all of our geriatric residents to ensure that each resident has the finest quality of care and quality of life while residing at the Schulman and Schachne Institute. Who is Eligible? The Schulman and Schachne Institute admits residents for geriatric skilled nursing care treatment when they are over the acute phase of their illness.  We also admit geriatric residents for short-term rehabilitation and hospice care.  Residents can be referred by the hospital or by an outside physician.  Self-referrals are also accepted.  For information about eligibility and admission, call (718) 240-6903.
