Welcome to the Department of Social Work at Brookdale University Hospital and Medical Center.

When illness occurs, individuals and families often need help dealing with psychological, social and financial matters.  They may need help with their hospitalization or with planning for their care upon leaving the hospital.  With the assistance of Brookdale's Case Management Department, patients can better cope with the emotional and social stresses of being ill.

Services include:

Information and Referral

• Practical help with emergency needs.
• Referral to outside agencies that can help with problems such as  child care, housing or health insurance.

Counseling and Psychotherapy

• Individual and group counseling for patients and their families.
• Help in dealing with chronic or terminal illness.
• Crisis intervention counseling.
• Support groups including cancer survival, bereavement, parenting, problems of aging, and alcohol and drug abuse.

Training and Community Work

Brookdale's Case Management department trains social workers and other hospital personnel.  Seminars and in-service training on child abuse, foster care, adoption, victimization, and other health-related topics are provided on a regular basis.  In addition, the department works closely with community agencies to assure continuity of care for patients.

The Case Management department is located on the third floor of the Strausberg building.  You can contact a social worker in the following ways:

• A social worker may visit routinely to determine whether you need help.
• You or your family may request the services of a social worker.
• A doctor or another member of the healthcare team may ask that a social worker see you.
• Social workers are assigned to each of the inpatient units as well as the designated outpatient service.
• To contact your social worker, either visit the nursing station where the patient is located and request to see a social worker or call the Case Management Department directly.
